Topical Glucosamine / Chondroitin Cream Relieves Knee Pain
by Heidi Splete

A topical cream containing glucosamine and chondroitin relieved pain in patients with osteoarthritis of the knee, said Marc M. Cohen and his colleagues at Monash University, Clayton, Australia.

Since oral glucosamine is subject to degradation by the liver, the researchers hypothesized that topical administration would have greater bioavailability.

In a randomized trial, 32 patients with knee osteoarthritis (OA) applied as needed the topical cream containing glucosamine sulfate 30 mg/g, chondroitin sulfate 50 mg/g, shark cartilage 140 mg/g, camphor, and peppermint oil.

Another 29 patients used a placebo cream that contained a small amount of peppermint oil to smell like the active cream.

After 4 weeks of therapy, visual analog scale pain scores in the test group decreased from 6.1 cm at baseline to 3.5 cm, while the placebo group scores decreased from 6.2 cm to 4.8 cm.

After 8 weeks, the visual analog pain scores had decreased to 2.8 cm in the test group, compared with 4.6 cm in the placebo group (J. Rheumatol. [30]:523-28, 2003).

The test patients used approximately 5.5 tubes of cream during the course of the study, and applied it to their knees approximately 2.5 times per day. The estimated daily topical dosage was 300 mg of glucosamine sulfate and 780 mg of chondroitin sulfate.

If a transdermal absorption rate of 20%-40% is assumed, 60-120 mg of glucosamine and 156-300 mg of chondroitin reached the patients' joints.

Respiratory and skin reactions were the most common adverse events and occurred in three patients in each group, suggesting a lack of toxicity or side effects associated with the glucosamine/chondroitin cream.

Although the placebo group reported a significant reduction in pain, the difference between the groups was statistically significant. Future studies should focus on long-term use and the potential for subgroups of responders, the researchers said.
