Other Names: Palmier di l'Amérique du Nord (French); Sãgepalme, Zwergpalme (German); palmetta di Florida (Italian).
Description: A more or less stemless palm (about 2 m), with greyish green, fan-shaped leaves having thorny teeth along the margins of the leaf stalk. It has dense clusters of small flowers and oblong, single-seeded fruits ("berries") of about 30 mm long that turn black when they mature.
Origin: Southeastern USA. The plants form dense thickets and the fruits are mainly wild-harvested.
Uses and Properties: Fruits are used to treat urination problems associated with an enlarged prostate gland. The medication does not reduce the enlargement but only alleviates the symptoms: an increase in the frequency of urination (pollakiuria), urination at night (nycturia), as well as a weak urinary stream and a delayed onset of urination. A more advanced stage of BPH (stage II) (a condition ascribed to the accumulation of dihydrotestosterone in the prostate) is characterised by residual urine and a frequent urge to urinate.
Preparation and Dosage: A daily dose of 1 - 2 g of dried fruit or 320 mg of lipophilic extract is recommended. Solvent extracts, using hexane or ethanol (or liquid carbon dioxide) are most often used. These are included in various commercial preparations.
Active Ingredients: The seed oil is rich in short chain fatty acids (especially lauric acid) and their glycerides. Extracts also contain various phytosterols - sitosterol (both free and esterified forms), campesterol and cycloartenol.
Pharmacological Effects: Saw palmetto apparently inhibits the binding of dihydrotestosterone at androgen receptors, and the enzymes 5-alpha-reductase and aromatase (thereby preventing the male hormone testosterone from being converted into dehydrotestosrerone and oestrogen). These activities are usually ascribed to phytosterols and provide a base for the use of saw palmetto, pumpkin seeds, hypoxis corms and pygeum bark in BPH therapy. Palmetto extracts also have anti-inflammatory activity. Several controlled clinical studies have shown significant improvements in the symptoms of prostate adenoma as compared with placebo controls.
