Description:Crataegus species are shrubs or small trees (up to 10 m), with lobed leaves, thorny branches and white flowers, that emanate amines to attract pollinating insects, followed by red fruit. Two species (that readily hybridise) are mostly used for medicine, namely C. monogyna (deeply lobed leaves, single-seeded fruit) and C. bevigata (= C. oxyacantha) (shallowly lobed leaves, 2-3-seeded fruit). Furthermore, C. azarolus and C. pentagyna are used in European phytotherapy; C. cuneata and C. pinnatifolia in Chinese medicine.
Origin:C. laevigata: Europe; C. monogyna: Europe and Asia.
Parts Used: Dried leaves with flowers (Crataegifolium cum flore); flowers and fruits are sometimes used on their own.
Therapeutic Category: Cardiotonic.
Uses and Properties: Hawthorn products are used to treat cardiac insufficiency (NYHA stages I and II) and bradycard rhythm disorders.
Active Ingredients: The drug contains 1-3% oligomeric procyanidins, together with about 1-2% flavonoids, such as vitexin-rhamnoside, hyperoside and rutin as the main flavonoids. Also present are chlorogenic acid, caffeic acid, and triterpenes.
Pharmacological Effects: Procyanidins and flavonoids represent the medicinally important compounds because of their phenolic hydroxyl groups they can interfere with various enzymes and receptors, among them Na+, K+-ATPase, and ACE (angiotensin converting enzyme). Numerous studies have shown an increase in the strength of contraction and the stroke volume of the heart. Anti-arrhythmic effects and increases in coronary blood flow have been documented. Controlled, double-blind clinical studies have shown the efficacy of treating patients with NYHA II; some evidence suggests that it might also work for NYHA III.
Preparation and Dosage: In modern phytotherapy, defined special extracts are used. The German Commission E recommends a daily dose of 160 - 900 mg of hawthorn extract containing 4 - 30 mg total flavonoids and 30 - 160 mg of oligomeric procyanidins. More recently, a dose of 600 - 900 mg of extract per day was recommended. Hawthorn products are taken orally and are only effective if used foraminimum period of six weeks.
