Kava Kava Piper methysticum Forster f.
Family Names: Piperaceae.
Other Names: Kava, kava kava (French); Kawapfeffer, Rauschpfeffer (German); kava-kava (Italian).
Description: An evergreen woody, scrambling shrub of up to 3 m in height bearing large heart-shaped leaves and oblong clusters of inconspicuous flowers (male and female ones on different plants). The branched, fleshy rhizomes can weight up to 10 kg. Several other Piper species are of medicinal value.
Origin: Polynesia; apparently an ancient cultigen derived from P. wichmannii; it is grown in the western Pacific (mainly the islands of Fiji, Samoa, Tonga and Vanuata). Traditional kava drink was prepared by women and children chewing the roots and then mixing it with water to ferment.
Parts Used: Rhizomes and roots (Piperis methystici rhizoma).
Therapeutic category: Sedative (anxiolytic).
Uses and Properties: In modern phytotherapy, kava roots and extracts thereof are used mainly to treat sleep disturbances, anxiety and stress.
Preparation and Dosage: Crude herb or preparations equivalent to a daily dose of 60-210 mg of kava pyrones is recommended. The treatment should last no longer than three months.
Active Ingredients: Kava root contains styrylpyrones, also known as kavapyrones or kavalactones (at a level of at least 3.5% of dry weight in high quality product). The main compounds are kawain, methysticin, dihydrokawain, dihydromethysticin, yangonin and desmethoxy-yangonin.
Pharmacological Effects: Kavalactones bind to various neuroreceptors, especially GABA and doparnine receptors, and inhibit Na+-channels. These interactions explain the observed sedative, muscle relaxing, anti-convulsive, tranquillising and analgesic effects. Kava can be seen as the herbal alternative to synthetic anxiolytics such as benzodiazepines, with no evidence of physical or psychological dependency. Several controlled clinical trials have shown efficacy for kava to treat patients with anxiety, tension and agitation or psychosomatic menopausal complaints.
